For Parents:
The survey will be available from Feb. 18, 2012 through May 1, 2012.
Parents may log into this site to answer education surveys for their child's school. The survey can display questions in both English and Spanish. Your answers will remain anonymous.
You will be asked to create a username, but you are not required to use your real name.
Parents may log in and answer survey questions once per year. This will help us collect meaningful data. The system will log which survey you requested and which questions are part of the survey, but your answers are not tied to your name.
For Administration:
Administration may log into this site to add new surveys and review the results of surveys as data accumulates. Your school must have a general administration login prior to initial setup. You may obtain this login information by emailing Brian Salter at Brian.Salter@state.nm.us .
For those districts who used the site last year, all surveys have been brought forward into the new year. You have until Feb 18th to modify questions if necessary. On Feb 18th all surveys will go live (begin the survey collection window). If you require more time to modify questions, please email Brian Salter to change the go live date. On Feb 18th you will also have the ability to print one page versions of the survey to send home with students.
Districts and schools are responsible for keying paper surveys into the site.
Feb 6 - Feb 17, 2012 -- Modifications of District and School questions.
Feb 18 - May 1, 2012 -- Survey collection window.
May 2 - May 11, 2012 -- Data review by school officials.
May 14th, 2012 -- Data certified for all reporting.
After the initial login, your main site administrator may add additional logins for support staff. If you were given a password/login from your local administrator and need assistance, you should contact your local site administrator before contacting the state offices.

Challenge Exam Registration Form 2012
The Clovis Municipal Schools will be administering three Challenge Exams on June 25, 2012 for any student who believes he/she can demonstrate proficiency in the essential knowledge and skills defined for the courses.
A passing score of no less than 85% is required to receive credit for these courses on the student’s high school transcript.

CMS 2012 Summer Reading Program
Welcome to the Clovis Municipal Schools Summer Reading Program. We want you to continue reading over the summer, so we have arranged for you to receive a summertime treat coupon when you bring in a completed reading log!
Here’s what you need to do:
- List the books you read that are on your reading level on the log on the back of this page.
- Bring completed logs to the Central Office at 1009 Main Street to redeem for a summer treat. We will accept logs and distribute treat coupons while supplies last from June 4 –
August 9. (Central Office will be open from 8-5 Monday through Thursday over the summer.)
- If you need more reading logs, you can get them at your school before it closes for the summer or from Central Office.
- Keep reading all summer! It’s fun, and it prepares you for next year!
