- Complete a substitute Application and Criminal History Affidavit - Answer every question completely. Incomplete applications WILL NOT BE ACCEPTED.
- Substitute educational assistants, secretary, and custodians require a copy of their High School Diploma, or GED. A copy of a college transcript is acceptable in lieu of a HS Diploma. IF YOU HAVE A BACHELOR'S DEGREE, YOU MUST PROVIDE US WITH AN OFFICIAL TRANSCRIPT REFLECTING YOUR DEGREE.

- W-4 (Be sure to sign)
- I-9 (Be sure to sign)
- Fingerprints - State Law requires anyone who works for the school to be fingerprinted. The Police Department or Sheriff's Office will do this and they charge $10. When you return the cards please attach a cashier's check or money order for $33.
- License (Substitute Teachers) - A license from the Public Education Department is required. Applications for the license will be completed upon receipt of fingerprints and money order. You will need a cashier's check or money order for $35 made payable to the Public Education Department to be attached when we send in the application. IF YOU POSSESS A NM TEACHING LICENSE, IT IS NOT NECESSARY TO OBTAIN A SUBSTITUTE LICENSE.
- Substitute Workshop - Attendance at a workshop is required for all substitutes before they begin work. Workshop dates are available in the Personnel Office or at the front desk.
- References for Substitutes - Be sure to give complete names, addresses and telephone numbers on the three names you give for references. We will call each reference and must have a minimum of 2 references back before you can work.
- Worker's Compensation Form - Read both pages; Sign, date and return only the second page.
- Substitutes may not work until file is complete.

- Complete the district's substitute teacher workshop
- Complete at least 60 hours of college-credit courses from a regionally accredited college or university and provide a transcript showing hours (a copy of the diploma is not acceptable)
- Observed teaching for 3 hours or more in this school district at the grade level of students in which you will serve
- Perform at least 3 hours of instructional services as a substitute teacher, in this district, within the past three school years.
- Complete or currently enrolled in an approved course or program from a regionally accredited college/university, where the course or program is structured to provide primary/secondary school teacher preparation.
- Engaged in paid employment during the past three years which, in my opinion, is relevant work or life experience. A statement from my employer showing dates of employment as well as a brief job description of duties/responsibilities will be provided if requested by the Assistant Superintendent of Personnel.

- Complete an approved teacher prep program from a regionally accredited college/university and will provide a transcript showing degree conferred (a copy of the diploma is not acceptable)
- Possess a current substitute or standard teaching license issued by an educator or licensure issuing agency of another state. I will provide my license that shows the state, license number, and effective dates.

- Complete the district's substitute teacher/ed assist workshop
- Submit a copy of your high school diploma or a college transcript in lieu of the high school diploma
- Submit a copy of your high school diploma or a college transcript in lieu of the high school diploma
- Complete the district's substitute food service workshop
- Submit a copy of your high school diploma or a college transcript in lieu of the high school diploma (if applicable)
